Social Services
The department aids patients and their families with the admission process to the Extended Care Facility, discharge planning, and referrals to community and out-of-area resources. The department helps residents arrange and attend appointments or functions outside the Nursing Home. The Social Worker acts as patient advocate and arranges for adult day care services when needed.
